Need Care?

Those that Faith in Action serves are called Care Receivers by our staff and volunteers. It is an honor to help our neighbors. We provide services to those that have enrolled as care receiver. To initiate the enrollment process, please:

  1. Review our offered Care Receiver Services
  2. Review our Care Receiver Requirements to determine eligibility
  3. Call our office at 309-827-7780 to begin the onboarding process

After receiving your Care Receiver request by phone, our Care Receiver Coordinator will call you to confirm your information and ask additional follow up questions. We will also perform a 60-90 minute in-home assessment. It can take several weeks to become a care receiver. Cancer and dialysis patients will be expedited to the best of our availability.

Care Receiver Services

Medical Transportation

Volunteers use their vehicles to transport care receivers to and from medical appointments. Volunteers do not attend medical appointments with the care receivers, but instead provide door-to-door drop-off and pick-up. Rides must be requested 7+ days in advance. Medical appointments also include physical therapy, mental health, dental, routine bloodwork or scans, etc. We are not able to be the responsible party for any procedure that requires anesthesia.

If there is no local provider for a medical specialist appointment, we can provide long-distance transportation four times a year.

Grocery Shopping

Volunteers are matched with a care receiver to provide transportation to the grocery store of their choice or food pantry 1-2 times a month. Volunteers might shop with or for a care receiver. Care receivers work with our grocery shopping coordinator to set up a shopping schedule.

Friendly Visits and Calls

Volunteers help brighten someone’s day with companionship and conversation. Our friendly visitors are paired with a care receiver for phone or in-person visits twice a month.

Light Home Maintenance

Volunteers do simple home repairs that do not require technical skills or equipment. Examples include changing a furnace filter or light bulb. Care receivers must use other Faith in Action Services before requesting home maintenance.

Quality-of-Life Rides

Once a month a care receiver may request a ride to the bank, post office, hair appointment, etc. To request a Quality-of-Life Ride you must be a current care receiver. These rides are filled only after medical and shopping rides are filled.

Care Receiver Eligibility

  • 60 years or age or older.
  • Ambulatory with minimal assistance. A care receiver may use a cane or walker, but we are unable to serve those with wheelchairs.
  • Pass a background check.
  • Live in our service area and live independently (we do not serve those in independent living, assisted living or skilled nursing facilities).
  • Our service area is Bloomington, Chenoa, Downs, Ellsworth, Heyworth, Hudson, LeRoy, Lexington, Normal and Towanda.
  • Live in a safe environment.
  • There are no income guidelines, we accept those of all economic backgrounds.
